Acton Town Medical Centre and Ground Floor Lanark Medical Centre- Patient Survey

Why we’re asking for your views

The GP contracst for ActonTown Medical Centre and Ground Floor Lanark Medical Centre has ended, and the current premises are no longer available for a new GP practice at this location.

We want your views to help us implement the change safely and fairly, including any support you may need, what matters most when choosing a new practice, and how we should communicate with you. If a practical opportunity arises to procure a new provider in future (for example if suitable premises become available), your answers will also help us understand what you would want from that service.

This survey will take 5-10 minutes to complete. Paper copies, translations and accessible formats are available and if you would like this survey in a different format please contact us by email: nhsnwl.communications.nwl@nhs.net

Closing date: 14th December
